Pre-Surgery / Dental Instructions Form

Please complete this form completely and accurately so that we can get to know you and your pet(s) before your visit. If this is an emergency or your pet requires immediate attention, please call us at 770-532-0491 for a faster response.

This document is to confirm that your pet is scheduled for surgery / dental. Our check in time is between 7:30 AM and 8:30 AM.

Please bring both the Pre-Anesthetic and the Surgical Consent forms with you when you drop off your pet the morning of surgery. By reading and signing both these forms, your check-in time will be greatly reduced.

Pre-Anesthetic blood work gives the Doctor an inside look at some of your pet’s vital internal organs. We are especially concerned with the health of the liver and kidneys as these organs help the body get rid of the medications used during anesthesia. We strongly recommend allowing us to complete this simple blood work to help insure the safety of your pet.
PRE-SURGERY / DENTAL CHECKLIST

1. Remove all food and water prior to midnight the night before your pet’s procedure.
2. Sign both the Surgical and Pre-Anesthetic Consent forms and bring them with your pet.
3. Give lots of extra pats and kisses!
All patients at Gainesville Veterinary Hospital are closely monitored while under anesthesia. This along with Pre-Anesthetic screening reduces the risk to the lowest level.
